Nebivolol is indicated for the treatment of essential hypertension and stable mild to moderate chronic heart failure.

Hypertension
Adults: 5 mg daily, preferably at the same time of the day.
Elderly (patients over 65 years): The recommended starting dose is 2.5 mg daily. The dose may be increased to 5 mg daily if needed.
Renal impairment: The recommended starting dose is 2.5 mg daily. The dose may be increased to 5 mg daily if needed.
Hepatic impairment: Data on patients with hepatic insufficiency or impaired liver function are limited. Therefore, the use of Nebivolol in these patients is contraindicated.
Chronic Heart Failure
The initial up-titration for chronic heart failure patients should be done according to the following steps at 1 to 2-week intervals based on the patient's tolerability:
Start with 1.25 mg daily.
Increase to 2.5 mg once daily.
Then increase to 5 mg once daily.
Finally, increase to 10 mg once daily (maximum recommended dose).
Use in Children: The safety and efficacy of Nebivolol in children have not been demonstrated.

Nebivolol is contraindicated in patients with severe bradycardia, heart block greater than first degree, cardiogenic shock, sick sinus syndrome, severe hepatic impairment, or those who are hypersensitive to any component of this product.

The most common side effects are headache, nausea, and bradycardia.

Patients with coronary artery disease treated with Nebivolol should be advised against abrupt discontinuation of therapy. Caution should be exercised in patients with circulatory disorders, first-degree heart block, anesthesia, diabetes, hyperthyroidism, chronic obstructive pulmonary disease, and allergen sensitivity.

Nebivolol is contraindicated in pregnancy and lactation.

As Nebivolol metabolism involves the CYP2D6 isoenzyme, co-administration with substances inhibiting this enzyme—especially paroxetine, fluoxetine, thioridazine, and quinidine—may lead to increased plasma levels of Nebivolol, which may increase the risk of excessive bradycardia and adverse events.

Symptoms of overdose include bradycardia, hypotension, bronchospasm, and cardiac insufficiency.

Store at a temperature not exceeding 30°C in a dry place. Protect from light and moisture.
